{"id":13305,"date":"2022-07-18T17:15:51","date_gmt":"2022-07-18T14:15:51","guid":{"rendered":"https:\/\/facias.ro\/?p=13305"},"modified":"2022-10-12T13:44:37","modified_gmt":"2022-10-12T10:44:37","slug":"obiectiv-monitorizarea-continua-transparenta-si-diminuarea-infectiilor-intraspitalicesti","status":"publish","type":"post","link":"https:\/\/facias.ro\/eng\/obiectiv-monitorizarea-continua-transparenta-si-diminuarea-infectiilor-intraspitalicesti\/","title":{"rendered":"Objective: continuous, transparent monitoring and reduction of intrahospital infections, state at the beginning of the project"},"content":{"rendered":"\r\n<p class=\"wp-block-paragraph\"><strong><em><u>Descrierea necesit\u0103\u021bii solu\u021bion\u0103rii<\/u><\/em><\/strong><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">\u00cen perioada aprilie 2021-februarie 2022, FACIAS a derulat proiectul \u201dFii Informat, nu abuzat!\u201d.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Aceste probleme au fost relevate, cu prec\u0103dere, din cele cca. 1000 r\u0103spunsuri \u00eenregistrate la Chestionarul FACIAS \u0219i din cele peste 3000 mesaje primite.\u201d<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Pentru componenta S\u0103n\u0103tate,\u00a0 una din cele cinci mari probleme semnalate pentru care ni s-a solicitat suportul din partea cet\u0103\u021benilor vizeaz\u0103:<\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Pericolul generat de infec\u021biile intraspitalice\u0219ti asupra pacien\u021bilor \u0219i faptul c\u0103 ei nu g\u0103sesc u\u0219or informa\u021bii pe tem\u0103, \u00een leg\u0103tur\u0103 cu spitalele la care merg, dar nici ce au de f\u0103cut pentru prejudiciile generate.<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong>Informa\u021bii la zi (oficiale):<\/strong><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong><u>Etapele procesului de identificare \u0219i raportare infec\u021bii <\/u><\/strong><strong><u>intraspitalice\u0219ti:<\/u><\/strong><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Autorit\u0103\u021bi competente: La nivelul fiec\u0103rei unit\u0103\u021bi spitalice\u0219ti, exist\u0103 o<strong> sec\u021bie dedicat\u0103<\/strong> (SPIAAM) care <strong>raporteaz\u0103 <\/strong>c\u0103tre direc\u0163ia de s\u0103n\u0103tate public\u0103 (DSP) jude\u0163ean\u0103 cazurile confirmate de infec\u021bii nosocomiale (denumite de lege IAAM). R\u0103spunderea legal\u0103 pentru raport\u0103ri revine conducerii unit\u0103\u021bii spitalice\u0219ti (Lege 3\/2021 art 30 alin 4). La r\u00e2ndul lor, <strong>DSP<\/strong>-urile locale raporteaz\u0103 c\u0103tre<strong> Institutul Na\u021bional de S\u0103n\u0103tate Public\u0103 (INSP)<\/strong>. ANMCS are obliga\u0163ia de a <strong>evalua <\/strong>activitatea de prevenire a IAAM. Potrivit legii, ar fi trebuit s\u0103 se \u00eenfiin\u021beze, \u00een termen de \u0219ase luni de la implementarea Legii 3\/2021, Comitetul Na\u021bional pentru Prevenirea \u015fi Limitarea Infec\u0163iilor Asociate Asisten\u0163ei Medicale (CNPLIAAM). Informa\u021bii colectate:<\/strong><\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>CNPLIAAM, autoritatea de control, nu a fost \u00eenfiin\u021bat \u00eenc\u0103, de\u0219i au trecut peste 6 luni de la termen (Legea 3\/2021); cu toate acestea, pe mandatul ministrului S\u0103n\u0103t\u0103\u021bii Ioana Mih\u0103il\u0103 a existat draftul HG<a id=\"_ftnref1\" href=\"#_ftn1\"><sup>[1]<\/sup><\/a> privind \u00eenfiin\u021barea CNPLIAAM (Centrul Na\u021bional pentru Prevenirea \u0219i limitarea infec\u021biilor asociate asisten\u021bei medicale);<br \/><br \/><br \/>Posibile cauze pentru care multe spitale nu fac raport\u0103rile:<\/li>\r\n<li>nu au epidemiologi<\/li>\r\n<li>nu exist\u0103 sanc\u021biuni pentru neraportarea infec\u021biilor nosocomiale (ar fi trebuit prev\u0103zute \u00een Normele metodologice ale Legii 3\/2021)<\/li>\r\n<li>p\u00e2n\u0103 \u00een 2016 era un punct de penalizare pentru manageri dac\u0103 raportau adev\u0103rul cu privire la infec\u021biile nosocomiale\u201d (sursa:Vlad Voiculescu, fostul ministru al S\u0103n\u0103t\u0103\u021bii<a id=\"_ftnref2\" href=\"#_ftn2\"><sup>[2]<\/sup><\/a>, 2020)<\/li>\r\n<li>agresivitatea DSP de a amenda\/sanc\u021biona cazurile determin\u0103 re\u021binerea medicilor\/unit\u0103\u021bilor de a raporta (Sursa: Vasile Cepoi, pre\u0219edinte ANMCS<a id=\"_ftnref3\" href=\"#_ftn3\"><sup>[3]<\/sup><\/a> , 2019)<br \/><br \/><br \/><strong>Cazuri publice de pacien\u021bi deceda\u021bi sau afecta\u021bi de infec\u021bii nosocomiale<\/strong><\/li>\r\n<li>Un pacient, Paul Costea, afectat de IAAM, a fost desp\u0103gubit \u00een 2020 cu 40.000 euro de stat, iar o alt\u0103 pacient\u0103, Ileana Adriana Totolici se judeca cu Spitalul Bagdasar Arseni ().<\/li>\r\n<li>Alt caz, spital trimis \u00een judecat\u0103 pentru moartea a trei bebelu\u0219i suspecta\u021bi de infec\u021bii nosocomiale () \u00een 2021.\u00a0<\/li>\r\n<li>Jurnali\u0219tii de la <em>Libertatea<\/em> au investigat <strong>dou\u0103 cazuri<\/strong> cu:<strong> 14 persoane decedate<\/strong> \u00een urma contract\u0103rii infec\u021biilor nosocomiale \u00een sec\u021bia ATI a Spitalului Militar Central la finalul anului 2020 \u0219i cu al\u021bi\u00a0 <strong>cinci pacien\u021bi deceda\u021bi<\/strong> de la complica\u021bii \u00een urma infec\u021biilor nosocomiale dintr-un spital militar de campanie, \u00een aceea\u0219i perioad\u0103 ().\u00a0<\/li>\r\n<li>Un pacient afectat de atac cerebral <strong>a ajuns aproape de moarte \u00een<\/strong> 2021 dup\u0103 ce a contractat infec\u021bii nosocomiale (). Avem adresa de e-mail a so\u021biei sale \u0219i num\u0103rul ei de telefon, o putem contacta (<a href=\"mailto:recuperarenic@gmail.com\">recuperarenic@gmail.com<\/a> \/ 0730.021.119).\u00a0<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><em><u>Alte informa\u021bii utile:<\/u><\/em><\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Art. 654 alin. 2 legea 95\/2006 \u00a0 prevede c\u0103 personalul medical nu este r\u0103spunz\u0103tor pentru daunele \u015fi prejudiciile produse \u00een exercitarea profesiunii c\u00e2nd acestea se datoreaz\u0103\u2026infec\u0163iilor nosocomiale, efectelor adverse etc.<\/li>\r\n<li>\u00cen primul trimestru din anul 2020 unit\u0103\u021bile din Capital\u0103 nu au nimic.<\/li>\r\n<li>Normele metodologice existente anterior privind infec\u021biile nosocomiale (Ordin 916\/2006) au fost \u00eenlocuite de ordinul Ordin 1.101\/2016.\u00a0<\/li>\r\n<li>Nu exist\u0103 monitorizarea deceselor celor \u00eemboln\u0103vi\u021bi \u00een spitale, de\u0219i este de importan\u021b\u0103 stringent\u0103 ().<\/li>\r\n<li>Sindicali\u0219tii din spitale \u2013 nemul\u021bumi\u021bi c\u0103 nu exist\u0103 condi\u021bii care s\u0103 previn\u0103 r\u0103sp\u00e2ndirea de infec\u021bii nosocomiale \u00een spitalele din Rom\u00e2nia ()\u00a0<\/li>\r\n<li>Pre\u0219edintele Klaus Iohannis a declarat public c\u0103 va <strong>cere explica\u021bii privind situa\u021bia nedeclar\u0103rii infec\u021biilor nosocomiale<\/strong>, \u00een octombrie 2020 ().\u00a0<\/li>\r\n<li>Rom\u00e2nia era pe primul loc \u00een Europa \u00een 2010 la num\u0103rul infec\u021biilor nosocomiale<a id=\"_ftnref4\" href=\"#_ftn4\"><sup>[4]<\/sup><\/a>.<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong>I.3. Ce nu \u0219tim<\/strong><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Care este situa\u021bia real\u0103 a infec\u021biilor nosocomiale<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Dac\u0103 se \u0219tiu sau nu nr de decese cauzate (total sau partial) de IAAM<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Ce are de f\u0103cut un pacient infectat \u00een spital; care sunt c\u0103ile de atac?
