The holder has complete control over access to his own file and may see at any moment who and from where did one obtain the DES's medical data.<\/li>\r\n<li>The latest version added the ability for anybody who desires to anonymize their data stored in the DES; following anonymization, only statistical information that cannot be connected to a person is retained in the system. Because anonymization is irreversible, the DES holder no longer has the option of regaining the medical data that was recorded prior to the date of anonymization. However, prior to anonymization, the DES holder might save the information from the personal health file in its own file.<\/li>\r\n<li>In addition, a mechanism for escaping anonymization was developed, via which a person can opt to build a new DES, in which the medical data registered by doctors would be gathered, beginning on the day the decision to exit anonymization was made.<\/li>\r\n<li>Complete methods for anonymization\/exit anonymization can be found on the CNAS website\u00a0<a href=\"https:\/\/cnas.ro\/dosar-electronic-de-sanatate\/\">https:\/\/cnas.ro\/dosar-electronic-de-sanatate\/<\/a>\u00a0.<\/li>\r\n<li>Apart from the holder, the DES may be accessed by doctors with the patient's agreement to learn about the patient's medical history, allowing for faster and more accurate diagnosis, treatment, and patient monitoring.<\/li>\r\n<li>Dosarul are \u00een componen\u021b\u0103 o sec\u021biune de date medicale de urgen\u021b\u0103, care poate fi accesat\u0103 f\u0103r\u0103 acordul pacientului. Aceast\u0103 sec\u021biune cuprinde datele medicale relevante \u00een cazul unei urgen\u021be, documentele \u00eenregistrate \u00een ultimele 6 luni, precum \u0219i informa\u021biile\u00a0 pe care proprietarul dosarului le poate completa \u00een sec\u021biunea \u2018\u2019antecedente declarate de pacient\u201d.<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<h2 class=\"wp-block-heading\"><strong>I.2. INFORMATION COLLECTED FROM PRIMARY (HUMAN) SOURCES<\/strong><\/h2>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><em>Note: This section will be updated when new data is obtained.<\/em><\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li><strong><em>Source: medical-surgical staff<\/em><\/strong><\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Today, the following practice is common in hospitals:<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">The patient has been admitted to Hospital A. The patient's medical history is taken upon admission (investigations).<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Hospital A's staff can only see data from their database; they cannot see what the patient did in other hospitals (B,C,D...).<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">The patient is asked to bring paperwork from previous hospitalizations or doctors.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">At admission and discharge, but also along the way, medical personnel spend about 30-40% of their time on paperwork and bureaucratic procedures, rather than allocating all of their time to patient care\/treatment.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph translation-block\">The patient is given the Discharge Form upon discharge, which states: the patient presented himself at the hospital for...with the symptoms....\n\nHe was given\/performed....x, y, z operations\/Upon discharge, his condition is........ and they  are recommended....<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph translation-block\"><strong><em>REIMBURSEMENTS - <\/em><\/strong>expenses\/patient -  patient has no access to it (FACIAS encountered only one exception, the other days, during the research)<\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li><strong>Source: Patients<\/strong><\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">A form is given to the patient upon discharge from the hospital. When do you expect DESI to be functional and beneficial for its intended purpose?<\/li>\r\n<li class=\"translation-block\">What are the steps to be taken at this stage for a patient to be aware of the expenditure incurred by the State for the benefit of his\/her health?<\/li>\r\n<\/ol>\r\n\r\n\r\n\r\n<ol class=\"wp-block-list\" start=\"10\" type=\"1\">\r\n<li>Were sanctions imposed on medical service providers (with or without contracts with CNAS) for failing to comply with the legal provisions governing the registration of medical data in the DES?
